- Special Quotation. Base pricing assumes a capacity 5,000 subscription users and a minimum use term of one year at a base price of $30,000. In addition, a subscription fee of $1,000 plus a 20% participation fee relative to member generated fees is required. Primary license is for use by 5,000 member accounts. Member accounts can be priced at no less than $6 per month. Features are demonstrated at www.medlifecard.com. Database is re-branded to purchaser. Requires three year license. License fees are based on number of users and are currently based on the current minimum of 5,000 members. Subscription can be renewed at the then existing fair market value for successive like terms. Does not include Server firmware/software and security encryption. These are additional cost items and are independent services provided by the hosting site. System requirements are SQL Server. Capacity will be affected by bandwidth and volume of users. Database changes are special quotation.
